Skilled Nursing
The care of the homecare registered nurse includes comprehensive
nursing assessments as well as specialized care including hematology,
oncology, Infusion Therapy, enterostomal therapy, pediatrics,
cardiovascular care, psychiatric/mental health, diabetes care,
restorative nursing, and maternal child care.
The homecare nurse provides many services to patients including
teaching, medication management, wound care, physical assessment,
monitoring of chronic conditions, and reports to the physician
routinely regarding the patient’s condition and plan of
care for further orders.
Cardiac Care
The Heart at Home program provides care for a multitude of cardiac
conditions which might include patients experiencing cardiac
arrythmias, cardiomyopathy, congestive heart failure, hypertension,
pre/post open heart surgery, Chronic Obstructive Pulmonary Disease
(COPD) with cardiac involvement or post coronary angioplasty.
The cardiac nurses provide education and monitoring of vital
signs, pain, weight, symptoms and activities, medication instruction
and diet instruction. The registered nurse coordinates the involvement
of other professionals in the care of the cardiac patient to
include therapies, home health aides, and social service. The
use of Telehomecare
is a valuable adjunct to the "Heart at Home" program.
Pediatric Care
SUN Home Health provides comprehensive home healthcare to children
with chronic or acute health problems. This program promotes
family unit preservation and provides care to the child in a
familiar home setting with their parents involved. The pediatric
program includes the skill of the registered nurse, social workers,
rehabilitative therapies, and custom care services for supportive
and respite assistance. Skilled pediatric services might include
wound care, ostomy care, medication instruction, enteral feedings,
home infusion therapy, infant stimulation therapies, physical
or speech therapies, private duty care for technology dependent
children, and hospice pediatric services as needed.
Maternal Childcare
This program provides education and skilled intervention during
a patient’s prenatal and postpartum experience. The program
includes skilled nurses, social service and community program
resources. The skilled nursing care might include medication
or diet education or supervision, prenatal teaching, diabetic
teaching, monitoring of pregnancy induced hypertension, care
of substance abuse mothers, monitoring of pregnancy complications,
provision of care to the baby including home phototherapy, newborn
feeding problems, or newborn monitoring.
Mental Health Program
The psychiatric/mental health program answers an increasing
need in the mental health arena for alternatives to institutional
care. This program provides skilled nurses trained in mental
healthcare along with medical social services, occupational
therapists and home health aides to provide short-term intermittent
attention to patients needing such follow up after their discharge
from an acute care facility or outpatient program. The services
include medication supervision and instruction, case management
and coordination of mental health treatment with other medical
needs, family support, environmental and pyschosocial assessments,
as well as assessment of psychiatric and emotional status.
