If you had a hospital stay for a chronic or acute condition such as congestive heart failure, a heart attack, pneumonia or diabetes and are over 65, studies show that you have a one in five chance to be hospitalized again within 30 days. You are not alone in your quest to stay out of the hospital and studies show that proper home support and education can greatly reduce the need for you to return to the hospital.

SUN Home Health and Hospice is an expert at helping patients STAY AT HOME. In fact we rank in the top 10% nationwide of all homecare providers in preventing re-hospitalizations. We want to help you STAY AT HOME in your familiar environment with your loved ones and have created the STAY AT HOME program to help you meet that goal. By joining this free program you will receive the following benefits:

• We will provide you with health education and beneficial tips to help you live safely in your home. 
• We will make you aware of additional community resources that you may qualify for that will help you STAY AT HOME. 
• We will provide you with access to nurses in SUN Home’s Information and Referral Center (IRC) 24 hours a day to answer your healthcare questions.
